Important Medical Information

For those who may not be aware, children in the NYC school system cannot have any medications,creams,ointments or drops in school without a completed MAF (504) NYC Form. This Form must be renewed each School Year.

This is very important information for any parent that has a child on medication or may be on medication for the next school year.

All current medication must be picked up by Tuesday June 26th, 2012.  I will not be here on the last day of school and the medical room will be locked.

Attached are the 504 Medication Forms ( you can also get them from the DOE web site under Offices and then Health).

The front form needs to be completely filled out by the Doctor and the back signed by the parent.

Please also send in a small current photo of your child.

All Students new to NYC Public Schools, and all 4year olds and 5 year olds MUST also submit a NYC Physical Form (SH 205) to the Nurse at the beginning of the school year or your child can be scheduled to see the School Doctor with your permission, free of charge.

All forms are available in my office as well. You can pick one up when you come in and pick up your childs medication.
